What Is a p-tau217 Blood Test?
p-tau217 (phosphorylated tau at threonine 217) is a form of the tau protein that can be measured in blood. Researchers and clinicians study it as a biomarker associated with Alzheimer's disease-related brain changes, especially amyloid pathology.
A p-tau217 measurement is not a general test of memory, and it does not by itself measure the speed at which the brain removes waste proteins.
Why p-tau217 Matters in Alzheimer's Research
Phosphorylated tau biomarkers can carry information about biological changes associated with Alzheimer's disease. Research has evaluated p-tau217 alone and in combination with other markers, including amyloid-beta measures. Different assays and combinations can have different diagnostic performance.
The meaning of a result depends on the specific test, its validated thresholds, the reason testing was ordered, and the individual's clinical situation. A result should not be interpreted as a stand-alone diagnosis or prognosis.
Is There an FDA-Cleared Alzheimer's Blood Test?
Yes. In May 2025, the U.S. Food and Drug Administration cleared the Lumipulse G pTau217/β-Amyloid 1-42 Plasma Ratio test to aid evaluation of amyloid plaques in adults aged 55 and older who have signs and symptoms of Alzheimer's disease. This is a specific combination test with a defined intended use—not blanket clearance for all p-tau217 assays or routine screening of people without symptoms.
The Alzheimer's Association's 2025 blood-biomarker guideline addresses use in the diagnostic workup of people with cognitive impairment in specialized care. It does not establish general population preventive screening of asymptomatic adults.
What Does a p-tau217 Result Not Tell You?
- It does not directly measure autophagic activity or glymphatic flow.
- It does not independently show that amyloid or tau is being cleared faster or slower.
- It does not establish that a treatment or lifestyle intervention prevents Alzheimer's disease.
- Its interpretation cannot be transferred automatically between different assays or laboratories.
How p-tau217 Relates to BAT Levels
BAT Levels is BATWatch terminology for considering beta-amyloid and tau biomarkers within a longitudinal brain-health monitoring framework. BAT Testing discusses the measurement of such markers, and BATScore describes a proposed scoring interpretation.
The BATWatch framework is distinct from independently validated assay claims. Measuring p-tau217 in blood does not by itself validate a proprietary brain-clearance score or prove that a biological intervention changes clearance efficiency.
